What we discuss first
Be ready to describe when the shoulder pain started, whether it followed an injury or a change in activity, and what is hard now: reaching overhead, lifting, reaching behind you, or sleeping on that side. Mention any neck pain, numbness or tingling into the arm, prior shoulder injuries or surgery, and your medical conditions and medications. That history helps a clinician decide whether hands-on care is appropriate or whether imaging or another evaluation should come first.
Care options and realistic expectations
Shoulder pain that comes from overworked or irritated muscles, or from how the shoulder blade, upper back, and neck move together, is commonly managed with hands-on care and targeted movement. NCCIH reports short-term benefit from massage for some people with shoulder pain; longer-term evidence is limited and results vary. Treatment choices should follow the assessment and your preferences rather than a fixed course of visits.
Share your health conditions and medications before treatment. Soft tissue work and joint mobilization can cause temporary soreness. Ask about expected benefits, risks, alternatives, and how progress will be assessed, and discuss the need for further care as your symptoms and range of motion change.

When to seek medical help first
Call emergency services for sudden shoulder pain with chest pressure, pain spreading to the jaw or arm, shortness of breath, or dizziness; these can be signs of a heart attack. Seek emergency care for a shoulder injury with deformity, severe swelling, or inability to move the arm. Get prompt medical assessment for shoulder pain with fever or redness, or for pain that does not improve after a few weeks of home care.
