What this website collects

The contact form asks for three things: your name, your email address, and your message. That’s all. There are no accounts, no logins, and nothing else on this site asks you for personal information.

Please keep health details off this form

The message box is ordinary email, not a secure medical channel, so please don’t use it for symptoms, history, or anything else clinical. We’ll cover all of that privately at your visit, where it belongs and where it’s properly protected.

What happens to it

Your message is emailed straight to the practice. It isn’t saved to a database on this website. It’s used to reply to you and for nothing else — no mailing lists, no sharing with anyone, and never sold.

Analytics

This site uses Google Analytics for basic, aggregate traffic measurement only — which pages people visit and roughly how often. It doesn’t tell us who you are. There is no session recording and no chat widget on this site.

Booking

Appointment booking runs inside a scheduling system provided by Charm, shown in a frame on the contact page. Anything you enter there is handled by that system under its own privacy terms, not by this website.

Your clinical records

This notice covers the website only. Your health records at Mend Health LLC are handled separately under the practice’s own privacy practices, which Dr. Morrow will provide when you become a patient.
